The home health aide is a certified person who provides personal care, home management and other related home health support services in order to assist patients to continue living in their own environment. Care is provided by a home health aide when there are disruptions due to illness, disability, social disadvantage or other problems in the home.
The home health aide is under the direct supervision of the registered nurse.
The role of the scheduling coordinator is to coordinate coverage of home care patients. Coverage is based on the days/hours prescribed for the specific level of aide for consistency and continuity of care by an aide who is a suitable match for the patient. This includes scheduling coverage with a similarly suited aide, if possible, when the original aide is unavailable.
The supervising community field nurse ensures compliance with local, state and federal regulations, as well as accrediting agency standards. In addition, this position involves:
- Supervising and evaluating the home care nurse (both employed by Royal Care and contracted) and admission process
- Multidisciplinary coordination of the delivery of patient care services
- Facilitating communication with the discharge planners and staff nurses
