If you choose employee-only coverage, Royal Care pays 100% of the premium. In that case, all 3 plans cost you the same—nothing!
If you choose employee-plus children coverage, you must pay a monthly premium, which is different for each plan.

WHAT IF I DON’T WANT HEALTH COVERAGE FROM ROYAL CARE AT ALL?
If you don’t want any coverage from Royal Care, you must say so on waive coverage. If you waive coverage, you will be provided with a package of the other tax-free benefits, such as compensation for transportation.
If you don’t give us an election form showing that you are waiving coverage, we will automatically enroll you in employee-only coverage.

DO I GET TO CHOOSE BETWEEN THE HIGH, MEDIUM, AND LOW PLANS?
You get to choose whether to enroll and if you want employee-only or employee-plus-children coverage. If you enroll, your plan will be based on hours you worked during the previous month.
If you have at least 120 hours in a 4-week month and 150 hours in a 5-week month, you will be enrolled in the High plan the following month.
If you have less than 120 hours but at least 80 hours in a given month, you will be in the Medium plan the following month.
If you have less than 80 hours and more than 0 hours in a given month, you will be in the Low plan the following month.
If you have 0 hours in a given month, your coverage will end on the last day of that month.*
*If you do not work any hours in a given month, you may enroll in our Cobra plan

WHAT IF I CHANGE MY MIND ABOUT WANTING ROYAL CARE COVERAGE?
If you enroll in coverage now... You can drop coverage for the remainder of the year at any time. To do so, you must submit a new election form saying that you waive coverage.
If you waive coverage now... You will not have another chance to enroll in coverage until next year, unless you have a “special enrollment right.” Information about special enrollment rights is enclosed.

I ALREADY SENT IN ONE OF THESE FORMS. WHY DO I NEED TO DO IT AGAIN?
We have open enrollment every year. You should consider these changes and make a fresh decision. Remember, if we don’t hear from you, we will automatically enroll you in Royal Care’s health plan.
